DietitianChong Pty Ltd ACN 613 884 398 (DietitianChong) respects your right to privacy and this policy sets out how we collect and treat your personal information. The following privacy policy applies to all DietitianChong users, and conforms to Internet privacy standards. Personal information is information we hold which is identifiable as being about you and includes sensitive information such as health information.

Collection of your personal information

We only collect information from you that you voluntarily give us. We may collect the following types of personal information from you: name, address, phone number, email address, IP address of the computer you are using, information about the goods or services you have ordered, information from enquiries you have made, communications between us, financial information (such as credit card and debit card information, expiration date and ccv), your age and medical information. For some of our products and services, we may be required to collect and hold your sensitive information.

If you request for any special dietary requirements, you may also be required to provide your personal information such as gender, height and weight, existing medical conditions and any medically diagnosed allergies or food intolerances, in order for us to customise a menu plan for you.

We collect and store personal information received via correspondence from you. This may be via the telephone, email, mail, fax or directly through our website and through interaction with the DietitianChong team (including agents or contractors to DietitianChong such as delivery drivers).

Correspondence is recorded in order to provide service references and to assist in our team development and training.

We take every reasonable precaution possible to protect your personal information and follow generally accepted industry standards to protect the personal information submitted to us, both during transmission and once we receive it.

When credit card details are collected, we pass them onto our CommWeb provider in order to be processed as required. We never permanently store complete credit card details.
